教職員識別證申請表 ( Employee ID Card Application )

申請原因說明Information for application reason

※首次申請請勾選新發,不需繳費,需上傳大頭照(1~2)

For the initial application, please check the initial issuebox. Its free of charge, but a recent 1or 2-inch bust and hatless photo must be uploaded.

※如因(發卡後7日內感應不良、資料有誤、調換單位)等原因申請重製者,請勾選重製(換發並要更換照片者,請同時上傳大頭照) (換發期間舊卡將註銷)

In case a new card is required within seven days after the original one has been issued because of poor tracking quality, wrong data, unit transfer, etc., please check the renewed issuebox. The old one will be nullified simultaneously. The photo on the card could be changed if the new photo is uploaded accordingly.

※如因(遺失、卡片損毀、卡片掉色、改名字等等)申請換發者,請勾選繳費重製先需至勵學大樓1F自動繳費機完成繳費,費用250元,並上傳繳費證明(換發並要更換照片者,請同時上傳大頭照) (換發期間舊卡將註銷)

In case a new card is needed because of card loss, damage, color fading, or name change, etc., please check the paid renewed issuebox. The old one will be nullified simultaneously. The photo on the card could be changed if the new photo is uploaded accordingly.

※與附屬機構(附院、小港醫院、大同醫院及旗津醫院)合聘者,由附屬機構發卡,請向附屬機構人資單位申請。

For those who are jointly employed by affiliated hospitals, please turn to the hospitals HR Department for employee ID card application.

※附屬機構借調至本校任職人員請繼續使用原有識別證,無需再向學校申請新卡。具主治醫師身分教師兼任學校主管者,亦同。

Personnel temporarily transferred from affiliated hospitals please continue to use their original identification card. There is no need to apply for a new card from the School. The same applies to teachers who are attending physicians and school directors.

※兼任教師申請前,需先經簽呈核定。

Adjunct teachers may submit applications only through approved official documents. 


上傳檔案說明Information for photo upload

※大頭照上傳檔名請使用身分證字號(身分證英文首字大寫)

Please use your ID card No. (with the first English letter capitalized) as the file name of the photo to be uploaded.

※繳費證明檔名請使用(職號+繳費證明,如1050111繳費證明)

Please use your employee no. andpayment proofas the file name of the payment proof to be uploaded.

 

領卡說明Information for card pick-up

※製卡約需三個工作天(不含例假日),本室將以mail通知領取。

A new card will be ready for pick up within three business days.  Please wait for notice by email. 

※門禁出入設定請參考資訊系統/最新消息/公告 [重要]校園IC卡問題,若繳費重製需重新設定門禁。

For access control settings, please refer to information announced on KMU Information System (only in Mandarin).
