主旨:113學年度就讀研究所逐年進修申請表或期滿需申請延長計畫之繳交期限及注意事項,詳如說明,請查照。

說明:

一、經核准國內、外進修學位之本校教職員工(含專任教師合聘附屬機構醫事人員、臨床教師),於113927日(星期五)前提交「就讀研究所逐年進修申請表」與修業期間成績單,經校長核准方得繼續進修。除前開申請表,另應檢附文件如下:

(一)進修中、復學者:檢具學號之當學期修課時間表。

(二)已無修學分者,請提供已註冊收據。

(三)休學、退學者:檢附學校核准文件影本。

(四)畢業者:敬請提供畢業證書正本予人力資源室進行驗證並檢附畢業證書影本1份(請載註員工編號)

二、期滿需申請延長,應依下列期限提出申請:

(一)國內進修者:應於期滿前3個月提出,敘明延長理由,並檢附延長計畫、指導教授證明書及成績單等有關資料,由原審查單位審議。

(二)國外進修者:應於屆期前3個月提出申請,惟進修期間一年以上者,應於屆期前6個月提出申請,敘明延長理由,並檢附延長計畫、指導教授證明書或進修機構同意延長之證明及成績單(進修學位者)等有關資料,需經所屬單位主管簽註,並由原審議委員會審議,校長核定。

(三)本校專任教師合聘附屬機構醫事人員、臨床教師之國外進修相關申請資格、權利、義務、審查程序及成果評核等有關事項,依其相關規定辦理。惟仍應於本校人力資源室公告期間提交進修申請表及進修成果,以提本校教師發展委員會核備。

三、業經核准進修且已取得較高學位者之改敘,依本校「教職員工敘薪辦法」規定程序需另申請辦理。

四、其餘未盡事項請參閱本校「教職員工國外進修實施要點」及「教職員工國內進修實施要點」。

五、核准進修事項可於本校校務資訊系統T.3.1.03.個人進修研習查詢

六、相關表單請逕自人力資源室網站分眾導覽「專任教師」及「職員工」-「常用表單」-「進修」下載填寫。

Deadline for Submitting Applications for Yearly Study and Extensions in Graduate School for the 113th Academic Year

A. Submissions by University Faculty and Staff 

University faculty and staff (including medical staff and clinical faculty in affiliated hospital) approved for domestic or foreign degree study must submit the “Application for Yearly Study in Graduate School” and transcripts by Friday, September 27, 2024. The application must also attach the following documents:

(a) For those currently studying or resumption of study: Provide the current semester schedule and registration number.

(b) For those with no credits: Attach the registration receipt.

(c) For those on leave of absence or who have withdrawn: Provide a copy of the approved document from the university.

(d) For those Graduates: Provide the original graduation certificate to the Human Resources Office for verification and attach a copy of the certificate (include the employee number).

B. Extension Applications

(a) Domestic study: Submit the application 3 months prior to the expiration date, including the reason for the extension, extension plan, certificate from the supervising professor, and transcripts. The application should be reviewed by the original examining unit.

(b) Overseas study: Submit the application 3 months before the term of study begins. If the study period exceeds one year, apply 6 months in advance, including the reason for the extension, extension plan, certificate from the supervising professor or institution, and transcripts. The application must be signed by the unit supervisor, reviewed by the original examination committee, and approved by the university president.

(c) Medical Staff and Clinical Faculty in Affiliated Hospital: Applications and results should be processed according to the regulations of the affiliated organizations. However, the form and study result still need to be submitted during the Human Resources Office’s announcement period for approval by the Teacher Development Committee.

C. Those who obtain a higher degree must apply separately for a payroll adjustment in accordance with the Regulations on the Remuneration Criteria for Faculty and Staff.

D. The remaining issues will be handled in accordance with the "Implementation Guidelines for the Domestic and Overseas Education of Faculty and Staff".

E. Approved Continuing Education can be accessed through the Campus Information System T.3.1.03 under the "Personal Continuing Education " section.

F. Relevant forms can be downloaded directly from the Human Resources Office website under the "Full-time Faculty" and "Staff" sections in the "Common Forms" - " Continuing Education " category.
